All The Things You Need to Know about SMMR 

Southern Maryland Myofascial Release (SMMR) is a  CASH PAY Practice 

What this means for you:  


  • You will pay IN FULL for your care at the time of service in the form of cash, check, or credit card. 
  • You will owe a FLAT FEE of $150 for any appointment missed or canceled with less than 24 hours notice.  Any returned check will be charged a $30 fee. 
  • Southern Maryland Myofascial Release does NOT participate directly with ANY health insurance company. 
  • If you have "out-of-network" benefits for physical therapy you may get reimbursed, in part or full, by yor health insurance provider. 
  • You may submit a superbill, provided to you free of charge by our office, to your health insurance carrier for reimbursement.  Please notify us of your intention to do so. 
  • Reimbursement is NOT guaranteed by Southern Maryland Myofascial Release.  Please contact your health insurance carrier directly to fully understand your costs and benefits prior to scheduling an appointment. 
  • Good Faith Estimates are available upon request. 



Preparing For Your Sessions  



  • You will receive your initial evaluation intake forms and follow up forms to your inbox at the email you provide when scheduling.  Please check your spam box if you don't see them.  Forms will come to you from IntakeQ and should be completed 24 hours before your initial or re-evaluation sessions.  Forms not completed at home will need to be completed in office and take away from your treatment session time.   
  • You will receive a text reminder 2 days prior to, as well as a few hours before, your scheduled appointment.  Do your best to arrive on time for all appointments.  Any late arrivals will be billed in full for whatever time is remaining of the session.  Please remember that any appointment canceled with less than 24 hours notice will be billed to you IN FULL.  
  • Come prepared for your appointment wearing, or being able to change into, shorts and a tank top or sports bra.  We need adequate visual and physical access to regions of the body for effective treatment both at the time of initial evaluation and subsequent appointments.  Thank you!
